John has over 45 years' practical and operational experience, and over 35 years' experience in major accident and incident investigations, in the mining, minerals exploration, petroleum, power generation, transport, waste management and construction industries.

Investigation training courses are based on the Reason Model (ICAM) of incident cause analysis, individually tailored to specific company requirements and applications.  Course content is industry specific with a specific focus on individual company needs.

John has been lead investigator in both domestic and international jurisdictions ensuring the investigation follows a structured and transparent approach to identify the underlying root causes, and to develop recommendations focused on risk prevention and mitigation strategies. He has had extensive involvement with major law firms in conducting invesigaions under legal privilege.  

 

He was employed for approximately 15 years with BHP Billiton, and enjoyed an active career in both the HR and HSE disciplines working in a variety of locations both domestic and international. This included six years as head of HSE for the BHP Billiton Global Exploration & Business Development Group.

 

During his time with BHP Billiton, John was a member of the team that developed the ICAM methodology of serious incident investigation, as well as authoring a study into more than 100 mobile equipment related fatalities at Australian mine sites over a ten-year period.
